We accept most insurance. It is your responsibility to contact your specific plan to make sure we are in network.
Copays:
If you have contracted with your insurance company to pay a copay, you are required to do so at the time of service.
Worker’s Compensation:
If your visit is a Work Comp case, you must have prior approval to be seen. If the case is pending, you will be considered self pay until it has been approved. Payment is due at the time of service.
No Surprise Act:
This Act went into effect on 1/1/22.
